For years, Scarpelli & Associates has been a staple of San Francisco's vibrant Castro District, an LGBTQI+ community. We don't advertise much, because we don't need to. Because we treat the hard-to-treat cases, and stick with our patients until they fully resolve. Bottom line : In a city full of rushed visits, productivity goals, aide heavy clinics and burnt-out clinicians, people know they get real care here.

We're a small team, and a small community clinic, quiet and sweet - every patient is known by name. Now we're looking for a PT to grow with us. We don't want a cog. What work looks like

  • One-on-one, hands-on care. You will either possess, or build deep skill in manual therapy and clinical reasoning, diagnostic eyes and hands. These are the things no algorithm or AI tool can do for a patient on the table. This clinic is known for training and possessing some of the best manual therapists in the Bay.

  • Practice as a true diagnostician. We work toward being the community's first stop, a primary care PT, not the last referral on the list.

  • Complex, meaningful cases. Physicians have been sending their complex patients who have already had physical therapy elsewhere to use for years. Your cases will include orthopedic, pelvic health, neurologic and everything in between, for a diverse community that deserves clinicians who see them fully.

Who you'll learn from

Our owner Dr. Cynthia Rose Koch, PT DPT OCS ERYT500, is a board-certified Orthopedic Clinical Specialist (OCS) and pelvic health certified physical therapist, who also holds the highest registered yoga training certification. She's deeply involved in advocacy through the APTA and CPTA. You'll get real mentorship from someone who is shaping the future of the profession, not checking boxes for a corporate residency program. Elizabeth Scarpelli, PT, OCS, FAAOMPT is the founder of the company and is currently retired, however, she offers mentorship on complex cases on a monthly basis. Development is not only optional, but advised, and regular consult/mentorship is built into your workload.

  • Cynthia is the owner and principal practitioner at Scarpelli Associates Physical Therapy

    Dr. Cynthia Rose Koch. PT/DPT, ERYT500

    Dr. Cynthia Koch, our current owner and principal PT hails from Canada and resides in San Francisco. She took over the practice of SAPT in 2025 after Liz’s retirement. She specializes in orthopedic spine, shoulder, foot and ankle conditions. She has taught yoga in San Francisco for over 15 years and is a certified pelvic floor physical therapist from Herman & Wallace. You may also find her singing in her 10-piece Soul Band Luvbomb around the city in a bright blue wig if you’re lucky!

  • Liz Scarpelli is the founder of Scarpelli & Associates Physical Therapy

    Elizabeth Scarpelli, PT, FAAOMPT

    Liz is the original founder of this 30 year old practice. She retired in 2025, and has been a therapist and teacher for over 35 years. She has mentored over 30 graduate PTs on the path to specialization in orthopedic manual physical therapy, including Dr. Cynthia Koch, current owner. She was a senior instructor for over 20 years in the Kaiser Hayward PT Residency and Fellowship program. Liz is an avid cyclist and still teaches workshops for PT’s, mentoring and consulting on difficult cases in our office.

  • Niki Naumchik, Office Manager/Client Services

    Niki ensures the office runs smoothly and efficiently. She enjoys working with clients to handle all aspects of scheduling, patient needs and navigating the logistics of insurance. She strives to make things as easy as possible for the client. She enjoys travel, photography and writing.
